Patients Revisiting the Hospital after a Long Interval (6 months or more)

  • If you are revisiting the hospital after more than 6 months without an appointment
Present your patient registration card, health insurance card/claimant certification for medical care, and referral letter to get an appointment at (3) Repeat Visitor Reception, and then go to the reception counter for the clinical section which you intend to consult. You need not fill in your information on the application form if you have previously had a medical examination at the hospital.
Note that you have to pay the first-visit fee plus a designated medical care fee (5,400 yen including tax) if you have no referral letter describing the history and outcome of treatment of your disease.

    * Patients who wish to consult Ophthalmology, Orthopaedic Surgery or Otorhinolaryngology-Head and Neck Surgery must bring a referral letter (with patient’s medical report). [Details]
