Procedure for Modifying Patient's Addresses, Names, Health Insurance Cards, etc.

If there is a change in one's address, name, health insurance card, etc., please register the change at (3) Repeat Visitor Reception prior to undergoing any examination. After the examination, please tell the staff at (5) Fee Calculation about the change.
Patients who have completed pre-registration for the deferred payment service should tell the staff at (7) Deferred Payment.
